This looks good. Is there a limit on the words? Because if not, I would just leave it as "Please let us know how we did."
Although Dr. Hartke is a good doctor, his nurse needs more experience. I ended up with a HUGE bruise on my arm from the nurse hitting an artery, instead of a vein, in BOTH of my arms. I do not recommend them.
